This form is used to formally document any change from the approved 'Good for Construction' (GFC) drawings or specifications. It is the official way to record a design modification, whether it comes from the client, consultant, or a site condition.

📍 When to use this template
  • When a site condition (like an unforeseen obstruction or soil strata difference) makes the original design impossible to build.
  • Whenever the Client or Architect issues a new instruction that changes the approved drawings or specifications.
  • Filled by the Site Engineer/Project Manager, reviewed by the PMC, and approved by the Client/Consultant before implementation.
