QA/QC Templates

QA (Quality Assurance) is process-focused — ensuring the systems exist to deliver quality. Activities: writing method statements, training workers, inspection test plans (ITP), internal audits, management reviews. QC (Quality Control) is product-focused — actually testing and inspecting the work. Activities: cube tests, dimensional checks, finish inspection, material test reports. QA prevents defects; QC detects defects. A good project has both — strong processes (QA) plus thorough inspection (QC). ISO 9001 is the global standard for quality management systems that governs both.

IS 456:2000 Clause 15 and IS 516:2021 together govern concrete quality testing. Cube test frequency per IS 456 Clause 15.2.1: 1 set of 3 cubes per 1 m³ up to 5 m³, then 1 set per 5 m³ up to 25 m³, then 1 set per 25 m³ up to 50 m³, then 1 set per 50 m³ up to 100 m³, then 1 set per 100 m³. Cubes tested at 28 days (also 7 days for intermediate check). Acceptance criteria: individual cubes >0.85×fck + 3 MPa; mean of 4 consecutive cubes >fck + 3 MPa (M15-M20) or fck + 4 MPa (M25+).

An ITP is a document that lists every inspection and test required for a specific work package, the frequency, the acceptance criteria, who carries it out, and who verifies/witnesses. Columns typically: activity, specification reference (IS code clause), inspection method, frequency, acceptance criteria, responsibility (contractor/consultant/third-party lab), record format (the test report template used), hold points (H = mandatory witness by consultant), review points (R = consultant reviews records). ITPs are the backbone of any QA/QC plan — a typical civil ITP might have 100-200 line items for an RCC building.

Hold Point (H): work cannot proceed past this point without the consultant/client present for inspection. Example: pre-pour rebar inspection before concrete is cast — if the consultant isn't there, the pour is delayed. Hold points are for critical, irreversible steps. Review Point (R) or Witness Point (W): consultant reviews records after the fact (not in real-time). Example: reviewing cube test reports in monthly meeting. Information Point (I): contractor informs consultant but no action required. Hold points are the strongest control; they're used sparingly for high-impact quality gates.

IS 1786:2008 — the TMT bar specification — has the acceptance criteria: Fe 415 UTS 485 MPa, yield 415 MPa, elongation 14.5% min; Fe 500 UTS 545 MPa, yield 500, elongation 12% min; Fe 500D UTS 565 MPa, yield 500, elongation 16% min (ductile, for seismic zones III-V). Inspection checklist: manufacturer's test certificate (MTC), ISI mark, rolling mark visible, bar size by weight (IS 1786 Table 3 tolerances), surface condition (no rust flakes), bend test per IS 1599, rebend test per IS 1786. Sample 1 set per 25-30 tonne delivery lot.

Our pre-pour checklist covers: (1) Rebar — diameter, spacing, cover, lap position, lap length, binding wire, cleanliness (no rust/oil/grease); (2) Formwork — alignment, plumb, level, tightness, release agent applied, inspection holes accessible; (3) Embedded items — plumbing sleeves, electrical conduits, anchor bolts, dowels — confirmed per drawings; (4) Construction joint preparation — roughened, cleaned, kept wet, pre-bonding agent applied; (5) Concrete placement — mix design approved, slump test equipment ready, cube mould ready, vibrator checked, curing material on standby; (6) Weather — no rain forecast, temperature 5-35°C. Signed by site engineer + consultant. Checklist is a hold point — pour doesn't start until it's green.

An NCR is raised when work is found not in conformance with specifications — cube test fail, rebar cover less than specified, dimensional deviation outside tolerance, wrong finish. NCR workflow: (1) Raise — site supervisor or consultant identifies non-conformance, writes NCR with description, references, photos, (2) Classify — minor (rework easy, no structural impact), major (rework required, schedule impact), critical (removal/redo), (3) Disposition — accept as-is (with concession), rework, repair, reject/remove, (4) Corrective action — fix the immediate issue, (5) Preventive action — fix the root cause (training, revised method, better supervision), (6) Close-out — consultant signs off. All NCRs tracked in the NCR Register with open/closed status.

Per IS 4031 (Methods of Physical Tests for Hydraulic Cement — 15 parts) and IS 4032 (Chemical Analysis): (1) Fineness (Blaine's method, IS 4031 Part 2) — min 225 m²/kg for OPC 33, 225 for 43, 370 for 53; (2) Standard consistency (IS 4031 Part 4); (3) Setting time (initial not less than 30 min, final not more than 600 min); (4) Compressive strength of mortar cubes at 3, 7, 28 days per grade; (5) Soundness (Le Chatelier method, IS 4031 Part 3); (6) Heat of hydration (for low-heat cement); (7) Chloride content (max 0.05% per IS 456). Sample 1 per 100 tonnes of cement delivery.

IS 2386 — Methods of Test for Aggregates — has 8 parts: Part 1 (particle size & shape — sieve analysis, flakiness, elongation), Part 2 (estimation of deleterious materials), Part 3 (specific gravity, density, voids, water absorption, bulking), Part 4 (mechanical properties — abrasion, impact, crushing value), Part 5 (soundness), Part 6 (measuring mortar making properties), Part 7 (alkali-aggregate reactivity), Part 8 (petrographic examination). IS 383:2016 — Coarse and Fine Aggregates for Concrete — specification: grading zones, deleterious limits, crushing value ≤30%, impact ≤30%, abrasion ≤30%. Test frequency: 1 set per 50 m³ of aggregate or per source change.

The templates follow ISO 9001:2015 principles (process approach, risk-based thinking, traceability, document control) — they can be used as-is within an ISO 9001 quality management system. Individual test format templates capture the data required by NABL (National Accreditation Board for Testing and Calibration Laboratories) for accredited lab reports: sample identification, test method reference (IS code + clause), environmental conditions, equipment calibration status, result and uncertainty, signatures. For formal NABL accreditation of your in-house lab, additional documentation (quality manual, procedures, SOPs, calibration records) is required beyond what the templates cover.

The templates are based on Indian Standards (IS) and MoRTH specifications — they work directly on Indian projects. For international projects using ASTM, EN, BS, or AS/NZS standards, the format (checklist/test/register structure) is universally applicable, but the specific acceptance criteria and test methods need to be mapped to the contract-specified code. Our templates show the IS code reference clearly so you can identify what to replace (e.g., "Concrete cube test per IS 516:2021" becomes "Concrete cube test per ASTM C39" on an ASTM-referenced project). The structural logic of the template doesn't change.
