**Sub-Centre** is the most peripheral health facility, covering 5,000 (plains) / 3,000 (hill / tribal) population — 1 ANM (Auxiliary Nurse Midwife) full-time + part-time MO. Building: 80 m², 4 rooms (consultation + delivery + dispensing + outreach). **PHC (Primary Health Centre)** covers 30,000 (plains) / 20,000 (hills) population — 1-3 medical officers + nurse + lab tech, 6-bed indoor + 24×7 emergency + delivery + immunisation. Building: 250-300 m², 8-10 rooms. **CHC (Community Health Centre)** is the first referral unit covering 1.20 lakh (plains) / 80k (hills) — 4-6 specialists (Gen Med + Surgery + OBG + Paeds + Anaesth + Dental + AYUSH), 30-bed indoor + Minor OT + Lab + Pharmacy + X-Ray. Building: 600-1,200 m² G+1, often a small campus with separate quarters.
